Please download, complete, and submit these forms prior to the start of the program.

You will be contacted prior to July 1, 2026, if you have been accepted into one of the two camp weeks. At which you will be provided with additional forms to fill out to be returned prior to the start date.

All three documents need to be filled out in order to be considered.
